Social and Economic Influences on Health

Many factors have an influence on our health. In addition to individual genetics and lifestyle choices, where we are born, grow, live, work and age also have an important influence on our health.

Determinants of health are the broad range of personal, social, economic and environmental factors that determine individual and population health. Access to health services is just one of the determinants that impact your health status.

 

At Mentor Clinic, our goal is to provide access to as many resources as possible to assist you in taking charge of your health journey.  There are unlimited resources available on the internet, so the ones below are those preferred and recommended by our team members.  These will be updated on a regular basis.  

The main determinants of health include:

  6. Social supports and coping skills

Online Appointment Booking
    • Follow the online booking link sent to you by our intake staff. 

    • Enter your first and last name, date of birth, and health card number. 

    • Pick from the list of providers available that day.

    • Pick your appointment type. 

    • Pick a date and time for the appointment. 

    • After inputting your email check your email to make sure it’s spelled correctly and book your appointment! 

    • You will receive a confirmation email with your appointment details. If you need to cancel or reschedule, use the link in the confirmation email or call one of our admin staff at 1-800-410-6672.

  • Mentor Clinics online booking system requires a perfect match between the name, birth date, and health card number to proceed. If the information in your health record and what you enter on the booking form doesn’t match, it may not let you book. We recommend double-checking spelling and your health card number when attempting to book. 

     

    If you are still unable to book online and you’re positive your info is correct, please call our admin team at 1-800-410-6672 to review the information that they have on file for you. 

     

    Please note, you need to be a registered patient at Mentor Clinic to book an appointment.

    • Mentor Clinic uses text and email reminders to provide convenient access to your appointment details, allowing you to easily confirm or cancel as needed.

    • The contact information used for these reminders is taken from your records, based on the phone number you provided during intake and you can call us at any time to opt out.

    • By providing this information, you consented to its use for appointment-related communications.

    • Your clinician may use Mikata, our AI tool, to assist with charting.

    • This helps your provider focus more on you and less on note-taking.

    • This tool will create a summary of your visit which will be sent to you so you have a record of what was discussed.

    • Mikata complies with all provincial and federal privacy regulations.

    • Your consent will always be obtained before using Mikata.

Types of Providers
  • Depending on your needs, you may first see one of our Family Practice nurses for services such as pap smears, well baby visits, vaccinations or general education.

     

    You may see a Family Doctor or a Nurse Practitioner for your care.

    Sometimes you may not see your regular provider, if that provider is away unexpectedly, or is delayed, in which case you may see another provider.

  • We are a teaching facility, and we regularly have learners at our clinic. Any of our providers may have learners, so they could be medical students, nursing students, social work students, occupational therapy students, etc.

     

    You will be told in advance if a learner will be present, and you will be asked for your consent to have them participate in your care.

  • Student learners are supervised by licensed practitioners (either family doctors or nurse practitioners).

    • All Healthcare providers at Mentor Clinic share the same electronic medical record (EMR) system, where they enter notes about all interactions with you. Providers communicate through the EMR and in person to share information about your care.

Types of Appointments
  • We have appointments set aside each day for same day urgent appointments for Mentor Clinic patients only. Please call as early as possible in the day if you have an urgent medical issue.

  • Appointment Cancellations/Rebooking - Please give us at least 1 Business day's notice if you need to cancel or reschedule an appointment with ANY of our providers. If you simply do not show up, that provider is unable to see another patient in that time slot.

    (Example: if you have an appointment on Monday, you must cancel on the Friday before, as early in the day as possible).

  • There is a charge of missed appointments. Please see our services section for more information and up-to-date prices. Please call and give us as much notice as possible if you cannot attend your appointment, so that we can give your appointment time to another patient. Please note that if you miss three consecutive appointments, you could be removed from this clinic, or no longer able to book appointments in advance.

  • Please call to book an appointment with one of our providers. They may be able to provide refills over the telephone.

Patient Rights and Responsibilities
  • As a patient, you have the right to:

    • Receive quality care that is respectful, safe, and appropriate for your needs.

    • Be informed of your diagnosis, treatment options, risks, benefits, and alternatives.

    • Participate in decisions about your care and give informed consent or refuse treatment.

    • Ask questions and express your opinions, preferences, and concerns.

    • Access your medical records and request copies or corrections.

    • Seek a second opinion or a referral to another provider or facility.

    • Provide feedback about our services, either positive or negative, without fear of reprisal.

    • Expect that your personal information and privacy will be protected and respected.

  • As a patient, you have the responsibility to:

    • Provide accurate and complete information about your health history, symptoms, and medications.

    • Follow the treatment plan agreed upon with your provider and inform them of any changes or problems.

    • Ask lots of questions and engage in decisions about your care.

    • Take responsibility for the care and maintenance of your own health.

    • Take your medications as prescribed and report any side effects or adverse reactions.

    • Keep your appointments or cancel or reschedule them in advance. Avoid last minute cancellations or no-shows.

    • Pay for any services that are not covered by your insurance or that you have agreed to pay for.

    • Respect the rights, privacy and property of other patients and staff.

    • Cooperate with our providers and staff and follow our policies and rules.

Uninsured Services
  • We bill you for uninsured services because we need to cover the cost of providing the service, plus office and staffing costs.

  • Please see our Services page for information about the services offered at our clinic, and which ones have extra fees.

  • The fees from all Uninsured Services go into our Patient Support Fund, which we use to assist patients in need of emergency support with items such as food cards, baby formula and diapers, transportation to appointments, assistance with healthcare aides such as walkers or canes, prescription medications, when no other community supports are available.

  • Tell our Admin team why you need an appointment when you make the booking. They will tell you right away if you will have to pay for any services. Your healthcare provider or the office staff will talk with you about a fee before providing you with a service. You will always receive needed services even if you cannot pay, so talk to your healthcare provider if you have a concern about the cost.

  • The price is based on how complicated the service is, the time and expertise needed, and the cost of supplies. Our prices align with those recommended by Doctors NS.

  • You can pay at the reception desk by cash, debit, credit card or e-transfer to email address payments@mentorclinic.ca

Services and Fees
  • We ask that you follow these policies to ensure a smooth and safe experience at our clinic: 

    • Please arrive on time for your appointment, arriving late may result in longer wait times or the rescheduling of your appointment. If you miss your appointment and do not cancel or reschedule with at least 1 business day’s notice, you will need to pay the Missed Appointment Fee before you can book another appointment with any of our providers.

    • To help us provide the best patient care possible, please bring your health card and a list of current medications, allergies, and medical conditions to each visit.

    • Inform us of any changes in your personal information, such as your address, phone number or email address, or when you get a new health card.  

    • Please be sure to respect the privacy and confidentiality of all patients and staff by not taking photos, videos or recording conversations without permission.

    • Help us maintain a safe and clean environment by not bringing weapons or pets into the clinic.  We have a water cooler available, and we encourage you to bring your own water bottle to stay hydrated! 

    • Please be polite and respectful to our providers and staff by not using inappropriate or threatening language when in the Clinic.

    • If you bring children to your appointment, help us maintain the safety of everyone in the clinic by not leaving your child unattended and keeping them in sight when in the waiting or exam rooms.

    • Please be aware of your conversations and the volume of your voice while in the clinic. We would appreciate it if you silence your phone and avoid using it in exam rooms or when meeting with a healthcare provider.

    • Please don't smoke, vape, or consume alcohol or drugs while in the Mentor Clinic.

  • We offer the following services at our clinic:

    • Primary care: We provide preventive care, chronic disease management, immunizations, care for health issues as they arise, referrals to specialists, requisitions for bloodwork and diagnostic imaging and prescriptions.

    • Urgent care: We treat minor injuries and illnesses that require immediate attention, such as cuts, sprains, infections, and allergic reactions.

    • Specialty care: We hope to have specialists who visit our clinic periodically to provide consultations and treatments for specific conditions, such as cardiology, dermatology, and psychiatry, or we will refer you to outside specialists.

    • Behavioral health: We offer counseling and therapy for mental health and substance use issues.

    • Diet Counselling – We offer the services of a Dietitian to aid with food-related issues.

    • Education and support: We provide health education and support groups for various topics, such as diabetes, smoking cessation, and weight management.

  • All required vaccinations for babies, children and adults are provided at our clinic.

    Some vaccines that are not covered by NS Health and must be paid by the patient. Some of these are are – Shingles Vaccine, Rabies vaccines. Please see our Services page for more information about pricing for these vaccines.

  • Vaccines may be given by our primary care providers (Physician or NP) or by any of our Nurses.

  • Note that we now have Family Practice nurses working in our clinic, and they are doing most of the routine pap smears. Please call and book an appointment with one of our nurses.

     

    Pap Smear – generally every 3 years from age 25 – age 70, please check with your provider.
